TELEHEALTH

Telehealth Consent & Policy

Draft for formal adoption/legal review.

Key information: Telehealth may be offered when clinically appropriate. It is not an emergency service, and some conditions require an in-person examination or a higher level of care.

How telehealth may be used

Nexus Health PC may provide clinically appropriate care through video and, when permitted and appropriate, audio communication. Telehealth may be used for evaluation, follow-up, medication review, care coordination, and other services appropriate for remote care.

Limitations and technology

Remote care may limit elements of a physical examination and may be affected by technology or connectivity. The treating clinician may recommend an in-person evaluation, testing, referral, or emergency care when needed.

Privacy and location

Patients should participate from a reasonably private setting and provide accurate identifying and location information when requested for safe care. Approved secure systems should be used when available.

Emergencies

Do not use telehealth, routine email, voicemail, or website communications for a medical emergency. Call 911 or seek immediate emergency medical care.

Consent

By participating in a telehealth encounter, the patient consents to remote care and acknowledges its potential benefits, limitations, and alternatives, subject to applicable law and the clinician's judgment. Consent may be withdrawn as permitted by law, without affecting actions already taken in reliance on it.
