Search FAQs
How do I request an appointment?
Use the appointment request workflow. Nexus reviews service-area, availability and applicable requirements before confirming an appointment.
Can a Home Health agency refer a patient?
Yes. Nexus Connect includes a professional referral pathway designed for Home Health and other approved referral sources.
Can I upload my ID and insurance card?
The Digital Check-In supports mobile capture. Live PHI transmission will remain disabled until the approved secure backend is connected.
Can I sign consent with my finger or pen?
Yes. The Digital Check-In includes a drawn electronic-signature interface for finger, stylus or mouse. Production e-signing will preserve the applicable consent version and audit information.
Does insurance verification guarantee coverage?
No. Eligibility or benefit verification is not a guarantee of payment, coverage, authorization or medical necessity.
Does the website diagnose my condition?
No. Website tools help with navigation, intake and education. Diagnosis and treatment decisions are made by qualified clinicians.
What should I do in an emergency?
Do not use website forms or chat. Call 911 or seek immediate emergency medical care.
How do I know whether my address is in the home-visit service area?
Review the Service Area section and submit an appointment request or contact Nexus to confirm a specific address. Home-visit availability is confirmed individually.
Is an appointment request a confirmed appointment?
No. Nexus must review the request and contact you to confirm the appointment.
Can I use telehealth for every medical problem?
No. Telehealth availability depends on the clinical situation, patient location, applicable requirements, and whether remote care is appropriate.
What should I prepare before my visit?
Have your photo ID, insurance information, current medication list, pharmacy information, relevant records or imaging reports, and referral information when applicable.
Can I send medical records to Nexus?
Use the Medical Records pathway for instructions. Sensitive records should not be sent through ordinary unsecured email.
Can another provider or facility refer me?
Yes. Nexus has professional referral pathways for physicians, Home Health agencies, facilities, case managers, and other care partners.
Can Nexus coordinate physical therapy or skilled nursing?
When clinically appropriate and applicable requirements are met, the clinician may coordinate or refer for services such as physical therapy, skilled nursing, or Home Health.
Can the website certify that I am homebound?
No. The website may collect relevant information, but any homebound determination or certification must be made and documented by the appropriate clinician under applicable requirements.
Will Nexus prescribe a specific pain medication or controlled substance?
No medication or controlled substance is guaranteed. Prescribing decisions are made individually by the clinician based on evaluation, safety, medical necessity, and applicable requirements.
Can I request an earlier appointment if one becomes available?
Contact Nexus or use the appointment workflow when available. An earlier opening is not guaranteed.
How do I request a copy of my records?
Use the Medical Records section for the appropriate records request or authorization process.
Can I cancel or reschedule online?
Use the scheduling/contact pathway for a cancellation or rescheduling request. The change is not final until confirmed by Nexus.
What if information on my insurance card is scanned incorrectly?
Any OCR or card-reading feature should present extracted information for confirmation. Patients or staff should correct errors before the information is relied upon.
Is the Nexus Assistant a medical chatbot?
No. It is designed for website navigation and approved administrative information. It should not diagnose, prescribe, or replace a clinician.
How is sensitive information handled on the website?
Production workflows involving protected health information require appropriate secure systems, access controls, vendor agreements where applicable, and other privacy and security safeguards. Ordinary public forms or email should not be used for sensitive information unless specifically configured for that purpose.
Where can I read Nexus policies and privacy information?
Open Patient Resources & Policies from the More menu for privacy, consent, telehealth, medication, financial, accessibility, website, and related information.
