Access and requests

Patients may request access to their health information and may exercise other applicable privacy rights through Nexus Health PC's designated records/privacy process. Identity verification and a valid authorization may be required for disclosures to third parties.

Release of information

A HIPAA authorization used for a disclosure that requires authorization must identify the information and parties involved and satisfy applicable requirements. Treatment, payment, and health care operations may be handled as permitted by law without using a general third-party release authorization.

Communication channels

Nexus Health PC may use telephone, voicemail, text, patient portal, and approved electronic systems for scheduling, prescriptions, care coordination, and billing as permitted by law and practice policy. Patients should not send sensitive medical information through ordinary email unless specifically directed to an approved secure process.

Response expectations

Electronic and routine communications are not monitored as emergency services. Patients should call 911 or seek immediate emergency care for emergencies.
