NEXUS DIGITAL CHECK-IN

One secure-ready workflow for intake, cards, records and consent.

Designed for phone, tablet or computer. Complete demographics, capture ID and insurance cards, prepare records, review consent and sign with a finger, stylus or mouse.

Build status: The interface works for review, including drawn signature and local file selection. Transmission/storage is intentionally disabled until Nexus connects an approved secure backend, identity/access controls, audit logging and applicable vendor agreements.

OCR can be connected later. Extracted values should always be confirmed by the patient/staff. Verification is not a guarantee of coverage or payment.

This intake supports clinician review; it does not diagnose, establish medical necessity, certify homebound status or guarantee treatment.

Detailed consent & signature

Complete the detailed consent before signing.

Nexus uses section-by-section choices so required acknowledgments and optional permissions are not hidden inside one blanket checkbox. Choose I Agree, I Do Not Agree, or I Don’t Know / I need clarification where applicable.

Signature comes last. The detailed consent page includes a large Apple Pencil/iPad pen, stylus, finger and mouse signature pad after the patient reviews all selections. “I Don’t Know” is not treated as consent.

Secure connection required before live submission. Production behavior: encrypted submission → secure Nexus work queue/EHR/document system → PHI-free notification → authorized staff sign in to review. Do not attach the complete intake package to ordinary email.
