PATIENT INFORMATION
Patient Rights & Practice Policies
Draft for legal/compliance review before final adoption.
Emergency care
Nexus Health PC does not provide emergency medical services. For a medical emergency, call 911 or seek immediate emergency care. Do not use email, website messages, or routine appointment channels for emergencies.
Telehealth
Telehealth may be used when clinically appropriate and permitted. It can have technical and clinical limitations, and an in-person evaluation may be recommended or required. Patients may ask questions about telehealth and available alternatives.
Medication safety
Medication decisions are individualized. When controlled substances or other high-risk medications are clinically considered, patients may be required to participate in appropriate monitoring, which may include prescription-monitoring-program review, toxicology testing, pill counts, pharmacy verification, follow-up visits, and other safeguards permitted by law and clinical policy. A request or prior prescription does not guarantee that a medication will be prescribed, continued, replaced, or refilled early.
Refills and safe storage
Patients are responsible for using medications as directed and safeguarding them from misuse, diversion, theft, or unauthorized access. Routine refill requests should be made during business hours. Controlled-medication requests are handled according to applicable law, clinical judgment, and practice policy.
Financial responsibility
Insurance coverage and benefits vary. Patients are responsible for applicable copayments, deductibles, coinsurance, and non-covered services. Nexus Health PC may bill insurance and accept assignment of benefits when appropriate and authorized. Patients should provide accurate insurance information and promptly report changes.
Good Faith Estimates
Patients who are uninsured or who do not plan to use insurance for scheduled care may have a federal right to a written Good Faith Estimate of expected charges. Contact Nexus Health PC for information before scheduled services.
Records and communication
Patients should provide complete and accurate medical, medication, substance-use, insurance, and treating-provider information and update the practice when information changes. Requests for records are handled under applicable federal and Illinois requirements.
Care coordination and optional programs
If Nexus Health PC separately offers and enrolls a patient in an applicable care-management or remote-monitoring program, the patient will receive program-specific information and consent as required. The listing of a program in a policy or consent document does not mean that Nexus Health PC currently offers that program.
Administrative requests
Forms, letters, disability/FMLA documentation, prior-authorization paperwork, and third-party requests may require additional processing, documentation, scheduling, or lawful fees. Availability is not guaranteed.
Professional relationship
Patients and the practice are expected to maintain respectful, safe, and truthful interactions. Treatment plans may be modified when clinically appropriate. Any termination of the clinician-patient relationship will be handled in accordance with applicable professional and legal requirements, including continuity-of-care obligations where applicable.
Privacy
See our Notice of Privacy Practices. Website visitors should not send protected health information through ordinary email.
Nexus Health PC will not rely on a website policy to waive patient rights that cannot lawfully be waived. Any arbitration, financial, consent, or medication agreement intended to create binding legal obligations should be separately reviewed by Illinois healthcare counsel before use.
