Insurance and patient responsibility

Nexus Health PC may bill participating insurance when applicable. Coverage, authorization, deductibles, copayments, coinsurance, and non-covered services vary by plan. Patients are responsible for providing accurate insurance information and for amounts assigned to them under their coverage or applicable payment arrangement.

Good Faith Estimates

Patients who do not have insurance or who do not plan to use insurance for scheduled care may have a right to a Good Faith Estimate of expected charges under federal law. Nexus Health PC will provide required notices and estimates through its billing workflow when applicable.

Administrative requests

Completion of forms, letters, disability or FMLA paperwork, third-party requests, and other administrative services may require separate processing or scheduling and may be subject to lawful fees when applicable. Medical-record reproduction charges, if any, will follow applicable requirements.
